Why tech companies make costly mistakes

Given that technology products pass through multiple departments before they are offered for sale, why do tech firms like Samsung continue to make costly mistakes that lead to substandard products? Take the example of the recent recall of Samsung smartphones due to explosive batteries.

The last Samsung incident was due to faulty batteries. They pretty much tried to get batteries from multiple companies and one of those messed it up, it's only a batch of Note 7's that's bad. Things like these happen and unfortunately can't be predicted.

Well stated. An excess number of cooks have indeed put sugar in Samsung's broth!
