Up until about 3-6 months ago, it was rare to have to complete a question to login as i think it used to be 4 or 5 failed attempts. It will now show after 2 failed attempts. It is to reduce the risk of someone malicious getting into users accounts. There are multiple lists of usernames/password combinations on the internet from sites previously hacked sites that sometimes get used. This was the case earlier this year when somebody attempted to flood the login page with alot of login attempts which is why I reduced the number of failed attempts allowed before having to complete a question aswell. It is hard to get around this question setup without someone purposefully writing some code to solve it which would be rare for somebody to do for a site our size.
Overall it is nothing to worry about and just a security method to confirm that the login attempt is being made by a human. Some users may have to complete it on their first or second attempts if someone (maliciously) has already exhausted some or all of the 2 failed attempts allowed. It is why we always say users should have a unique password for every site as these bad things do happen and can be successful if you reuse passwords.
If you are banned, you will be told so along with the ban duration and any reasoning we give. If you can't login then it is an issue somewhere probably down to an incorrect username or password being used, CAPS LOCK LEFT ON or just a mistype. If you still can't get in, you can then reset your password and you should be able to gain access to your account through that method.
